We now use a technique called the Glitch Rush or "grush." Basically you rush a character to normal Baal, and join a game in which the game's creator will finish Baal's quest for the first time. As long as you're partied and in A5 (the town works) you will finish the quest and be able to go on to Nightmare, even at level 1. The only thing that you 'need' to level for is to do the Ancients quest, which lets you go and fight Baal yourself.
You can do the same thing for Nightmare, and then to level from 1 to 60 in about an hour you do Chaos Sanctuary runs in hell difficulty. After 60 you can do some more chaos runs, or you can go do hell Baal runs untill 99.
